Daily dialysis care: recording key measurements for medical monitoring

Each day that I receive my dialysis care, I make notes during the session of various measurements as they happen. For instance, every 20 minutes my blood pressure is measured, so I record that, and the pulse at the time, and relative blood volume, and the hypotensive risk. At the end of dialysis, six key totals are recorded: eKt/V (Clearance), Blood Volume, Substituate Volume, Relative Blood Volume, Total UF (fluid removed), and final weight.

These are recorded in my notebook on each daily page. However, starting May 2024, I intend to add a line to a table on Daily dialysis records to record these six measurements. I hope that, in time, it will be useful to medical teams and me.
